Serenity Hospice and Home provides an Interdisciplinary Team of trained Hospice personnel to create and maintain a plan of care. The care plan is developed to meet each patients individual needs for pain management and symptom control. The team consists of a Medical Director, Nursing Services, Social Services, Bereavement Coordinator, Chaplain, specially trained Volunteers, and ancillary personnel as needed.
